1. Free of infectious diseases certificate and test results
2. Complete blood count
3. Liver Function Test (ALT, AST, GGT, AP, ALBUMIN, BILRUBIN)
4. Renal Function Test (Urea, Creatine, BUN, Electrolytes)
5. Lipid Profile (8-10 hours of fasting)
6. Fasting Blood Glucose (8-10 hours of fasting)
7. Urinalysis (Routine and Microscopy)
8. VDRL or RPR
9. HIV test
10. Hepatitus B
11. Hepatitus c
12. HGV (Clear orginal tracing with interpretation)
13. Chest X-Ray (High Quality, Identification embedded in the film with radiologist report)
14. Pure tone audiogram (Clear original tracing with audiologist/ENT intpreation)
15. Visual Acuity and Colour Vision (accurate assessment with eye specialist interpretation)
16. Other specialist report (if required – based upon medical assessment)
17. VO2 Max or stress test (Clear original tracing with interpretation)
18. Spirometry (Clear original tracing with interpretation)

The paperwork also mentions HGV so I am presuming (CLEAR ORIGINAL TRACING WITH INTERPRETATION).
